American Heart Association Social Impact Fund is accepting applications for sustainable, community-driven solutions to improve health disparities in Chicagoland
CHICAGO, March 5, 2024 – Last year, one in five households and one in four households with children in the Chicago metropolitan area were considered food insecure.[1]. As a social determinant of health, food insecurity can lead to many health problems such as heart disease and high blood pressure.[2].
Celebrating 100 years of lifesaving work, the American Heart Association today announced the addition of Grubhub Foundation to its social impact fund in Chicago. This funding injection, supported by Grubhub Community Fund, will enable local organizations to focus on technology-driven solutions to address systemic social issues that impact food security and economic development. Local Chicago organizations are asked to submit formal expressions of interest to the association’s Social Impact Fund through April 25, 2024. A contact point for organizations in the New York area is expected to open later this spring. Applicants can be at any stage of business and may be nonprofit or for-profit social enterprises. A virtual “Town Hall” information session will be held on March 13, 2024 at 10 a.m. Central. The Oscar and Kathy Munoz Pave It Forward Foundation established the Institute’s Social Impact Fund in Chicago in 2022. Donations from Muñoz and support from the Talman Family Office recently funded four of his organizations in Chicagoland:
- Care Advisors is a healthcare company that helps individuals quickly access social services and critical medical care by enabling interoperability and coordination of care between hospitals, clinical social workers in community-based organizations, and patients. It’s a technology company.
- Connect Care Hero (CCH) is a technology-enabled engagement platform for senior living that optimizes staff time, delivers curated health-focused content, and monitors age-related risks. Cope with isolation. CCH operates in more than 30 locations throughout Chicagoland and Southwest Florida.
- DistrictZero (DZ) is a digital emotional health platform aimed at promoting stronger social-emotional capabilities, helping individuals manage their emotions and make responsible decisions. DZ has successfully piloted the platform across U.S. K-12 schools, bilingual classrooms, private clinics, and corporate mental health support workflows.
- ProUnitas works with Title I schools to develop systems that address basic needs across social determinants of health and provide resources that students may need. The Houston-based organization recently expanded to Chicago.
